p/t Floating Support Worker, Somerset

Package: £10-12 per hour DOE

Responsible for:
1. Providing one-to-one support to a caseload of clients identified as vulnerable.
2. Completing Assessments of Need and Risk and implementing Support Plans to meet the identified needs.
3. Maintaining clear, accurate file records to demonstrate client work and actions, required as evidence by Supporting People.
4. Being and remaining well informed of all Supported Housing Policies and Procedures.

These responsibilities involve:
•Involving Service Users and taking a client led approach in all areas of service delivery
•Managing the risks associated with Lone Working
•Knowledge of the Supporting People Initiative
•Networking and promoting the support services provided by Supported Housing.
